Annual Proof of Residency

  • Each year, families must provide Annual Residency Verification documentation to the district. Those who do not wish to use the digital process can submit paperwork at Annual Residency Verification dates on Tuesday, March 5 from noon until  7:00 p.m., Wednesday, April 17 from 8:00 a.m. until noon and 4:00 p.m. until 8:00 p.m., and Saturday, May 4 from 8:00 a.m. until noon at their school. New students and incoming freshman must complete the Annual Residency Verification process through the traditional paper process.

    Category I (ONE Document Required)

    • Most recent property tax bill and proof of payment, e.g. canceled check or Form 1098 (homeowners)
    • Current mortgage statement or closing documents from home purchased within past TWO Months
    • Signed and dated lease and proof of last month's payment, e.g. canceled check or receipts (renters). Lease must be valid on the first day of the new school year.
    • Letter from manager and proof of last month's payment, e.g., canceled check or receipt (trailer park residents)

    Category II (THREE Documents Required)

    • Driver's License or State ID
    • Vehicle Registration
    • Most recent cable television bill
    • Current homeowners/renters insurance policy and premium payment receipt
    • Current Public Aid Card with Address
    • Most recent Gas Bill
    • Most recent Electric Bill
    • Most recent Water Bill

    **NEW** Digital Option for 2019-20 School Year

    A new online form submission process is being piloted this year to ease the process of turning in annual residency for returning students. The district is excited about the modernization of this process and appreciates families’ patience as we implement the new system. Once submitted, all documents will be reviewed by staff to assure authenticity. Parents interesting in submitting documents digitally should following the steps below:

    If using a Smartphone:

    1. Take a picture of the Category I and Category II Documents (Parent/Guardian Name and Address must be visible).
    2. Click on https://lf.d230.org/Forms/Residency.
    3. Fill out the form and upload the documents into the correct categories.
    4. Provide electronic signature at the bottom of the form and click submit.

    If using a computer (Desktop, laptop, etc.):

    1. Using a scanner or printer, scan the necessary Category I and Category II Documents and save to a location on your computer that you can access (Parent/Guardian Name and Address must be visible).
    2. Click on https://lf.d230.org/Forms/Residency.
    3. Fill out the form and upload the documents into the correct categories.
    4. Provide electronic signature at the bottom of the form and click submit
