INSTRUCTIONS TO UPLOAD HEALTH REQUIREMENTS  

Students entering 12th grade in the State of Illinois are required to have proof of the following immunization from their physician. 
Due by May 1, 2023

• A dose of Meningococcal conjugate vaccine on or after their 16th birthday. 

Acceptable documentation of meningococcal conjugate vaccination includes:

• Printout from your health care provider’s electronic medical record indicating the date(s) of vaccination OR
• Current Certificate of Child Health Examination signed by your healthcare provider indicating the date(s) of vaccination.
