    COVID-19 Vaccination Proof

    STUDENTS: Parents that wish to submit their child’s COVID-19 vaccination record can do so by clicking on “Online Forms” in the left navigation area of Skyward Family Access.
    STAFF: Staff that wish to submit their COVID-19 vaccination record can do so here.

    HEALTH FORMS & INFORMATION

    INCOMING FRESHMAN STUDENTS
    All incoming freshman are required to submit

    1. STUDENT EMERGENCY INFORMATION  Due by March 31, 2022 

    2. CERTIFICATE OF CHILD HEALTH EXAM  Due by May 1, 2022
      Health examination with proof of immunizations prior to the first day of school. This health examination must be completed by a MD, DO, APN or PA on or after 8/17/21.  The Health History section on page 2 must be completed and signed by the parent/guardian.

    3. DENTAL EXAMINATION  Due by May 1, 2022
      Dental examination must be completed by a licensed dentist on or after 11/15/21. 

    INCOMING SENIOR STUDENTS

    Students entering 12th grade in the State of Illinois are required to have proof of the following immunization from their physician - Due by May 1, 2022 

      • A dose of Meningococcal conjugate vaccine on or after their 16th birthday. 

    Acceptable documentation of meningococcal conjugate vaccination includes:

      • Printout from your health care provider’s electronic medical record indicating the date of vaccination OR
      • Current Certificate of Child Health Examination signed by your healthcare provider indicating the date of vaccination. 

    INCOMING TRANSFER STUDENTS

    All Transfer students must submit:

    STUDENTS WITH DAILY HEALTH NEEDS

      • Any medication required during the school day, including over-the-counter medication, requires a Medication Authorization Form and must be stored in the health office. 
      • Students with asthma, diabetes, epilepsy, and life threatening allergies require additional medical documentation.  

    SPORTS PHYSICAL EXAM

    Required for student athletes grades 10, 11, 12
