Sports Physicals

Required for student athletes grades 10, 11, 12

• All students wishing to participate in athletics must have an IHSA PreParticipation Physical Form on file with the athletic office. 
• All Sports Physicals should be sent to CHSD230 Athletic Physical Upload or visit your school's Athletic Department page for more information:
  • Sandburg HS Athletics Department 
  • Stagg HS Athletics Department
  • Andrew HS Athletics Department
